Patient Registration

If you are not currently a patient of the Practice, you live within our practice boundary and you wish to register, you simply need to call into the surgery for a registration pack.  Alternatively, please print off the documents below, complete them and bring them to the surgery at your convenience.

We are also required to confirm your registration details.  When you return the completed forms to us, please also bring along the following forms of ID: -

  1. Passport or birth certificate; and
  2. Driving license or recent utility bill which gives your current address.

Registering with Us if you are Homeless

Named Accountable GP

Once you have successfully registered with the Practice you will be allocated a named, accountable GP who will have overall responsibility for your care.  We will advise you who that will be.  Whilst you are entitled to consult any of the GPs in the Practice, we would encourage you to remain under the care of your named GP for continuity.  We will endeavour to accomodate any preference you have for your named accountable GP.  Please let us know if this is the case.
