Need help getting online?

We are now offering all patients an option of support in getting set up and using online apps. AskFirst and the NHS App allow you to complete tasks online without having to ring the surgery, like booking some appointments, looking at your medical record and test results, and requesting prescriptions.

By using the digital assistant, you will be able to access step by step guides on how to download and/or use functionalities within our online apps.

The NHS App will you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:

  • book or cancel appointments online
  • renew or order repeat prescriptions online
  • view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results
  • there is also record viewing including documents from 2022

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. More information including “how to” leaflets and videos of patients and why they are using GP online services are available at www.nhs.uk/gponlineservices

How to sign-up

For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up

The Practice now offers a number of services online including: -

  • booking an appointment
  • viewing aspects of your medical record, including test results
  • seeing vaccinations

To register for online services, you can self-register and identify through the NHS App.  This may not work with some devices and if this is the case you will need to attend the surgery with photo ID so Reception staff can verify your identity.  You will then be given the unique codes that will allow you to complete your registration.  Please use the form below if you havent been able to complete your request on your mobile, tablet or computer:

 

Patients over 16 years old can then see records, documents, vaccinations, test results, appointments and medication

Online access for 16 and over

 

We are able to offer proxy access so you can view multiple patients, this may be for children or adults that are in your care. Please download and fill in the relevant form that fits the criteria for the proxy access you are requesting. These are as follows.

Children aged up to 16 yrs proxy access will only be granted for prescriptions ordering, immunisation's records and appointment booking via the NHS application.

For more detailed access to the patient’s medical records, you can request this via rectorymeadowsurgery@nhs.net - we can then verify your request.

At Age 16 Proxy Access will be switched off. 

Please note that all requests will involve a thorough check of the medical record to ensure that the record is accurate and ready for viewing - this could take up to 10 working days.

You can access online services on the go by downloading the NHS App NHS App and your NHS account - NHS (www.nhs.uk) or downloading Patient Access App onto your mobile device.
